Daily Symptom Check

New Jersey Executive Order 192 and the New Jersey Restart Standards for Institutions of Higher Education require all students, faculty, researchers, and staff to complete a Daily Symptom Check report each day that they are scheduled or plan to enter any University building or worksite, including those on and off campus, except for their on-campus residence.

Web-based Form

Access the Daily Symptom Check web form through the TigerSafe app on your smart phone or directly on the web at https://tinyurl.com/PrincetonSymptomCheckApp. You will need to log in with your Princeton NetID and password, and complete DUO authentication. 

For the TigerSafe app, choose the Daily Symptom Check button to access the web-based form (instructions for downloading the TigerSafe app).

          

After answering the health questions, a green checkmark indicates that you are permitted to come to work on site or enter University buildings.

If your report symptoms that indicate the need to stay home, you will receive a message asking you to confirm your answers. After selecting OK, you will see a red circle, with a note informing that you must stay home and follow your departmental procedures to report your absence. Students are expected to follow instructions for contacting University Health Services, as appropriate. Messages and instructions for undergraduates, graduate students, and faculty and staff are customized for the constituency.

Failure to Complete the Daily Symptom Check

If you use your Princeton University ID card to enter a University building and you have not completed a Daily Symptom Check, you will receive an email reminding you to do so. If you do not do so, your non-compliance will be included in a report that goes to department representatives for faculty and staff, to the Graduate School for graduate students, and to the Office of the Dean of Undergraduate Students for undergraduates. Repeated non-compliance may result in loss of access to buildings or disciplinary action.

If you use your Princeton University ID card to enter a University building after receiving the notice to stay home, an University official will reach out to ensure that you leave campus immediately.

Paper Version

If you do not have a smart phone or internet access, you can use the paper version of the form.

Complete the form before coming to work on site. If all of your answers are in the green column, you are permitted to come to work. Bring your completed form to work with you and follow your supervisor or department instructions on what to do with them. If ANY of your answers are in the red column, you should not come to work. Keep the form with you and follow whatever procedures are in place for reporting an absence. Do not give the completed for to your supervisor or designee, since it contains health-related information that you do not have to share.

Keep a supply of paper forms with you at home. You can print the form yourself or obtain copies from your workplace.

Absence Management

Faculty, staff, and researchers who are advised not to come to campus may still be able to work from home. If you are unable to work remotely, you may utilize unused COVID-19 days, or other paid time including sick, vacation, personal days, and/or bundled time.
